Hello Anna,

I completed my QHHT training in December 2013, Sydney class.

A copy of your release form would be invaluable to me and very much appreciated. As I am currently practicing my QHHT technique I will definitely use this forum to obtain advice and guidance, therefore I will definitely wish to obtain my clients permission to disclose any information.

I got a release form from the school of hypnotherapy I graduated from that I use and I added the line that the insurance company required on the form. It would be interesting to see what forms others are using for QHHT.
